About Us - Care Philosophy
The mission of Berlin Health and Rehabilitation Center is to provide the finest quality health care, rehabilitative and support services to our residents and their families.
Our care approach is patient-centered and involves an integrated program of therapies that address the total well- being of our patients. For each resident, we create an individual care plan and assemble a care team that may include specialists from several areas. The goal is to design a course of treatment and therapy that best meets the needs of the resident.
Celebrating life! At Berlin Health and Rehabilitation Center, we look for ways to bring people together to share pleasant activities. Our annual harvest supper and summer festival are just two of the many celebrations and special events we host for our residents and their families.
Volunteers Welcome Volunteers are always welcome at Berlin Health and Rehab. Our volunteers help with activities, outings, meals and more. Interested in joining our wonderful group? Contact us by phone or e-mail for more info!
Special Outings We believe it is important for our residents to enjoy outings into the community for recreation, dining and shopping. Our schedule includes special trips to local attractions including fairs, parks, parades and more. Our fully- equipped, wheelchair-accessible van makes these outings possible for many residents.
